PROCEDURE OF ACTIVITIES IN CASE OF  
ILLNESS, TRAUMA OR DEATH  

Purpose:
Systematization of the process of decommissioning in case of trauma or illness as well as the process of repatriation of the dead.

Limited Liability Company “Elena Shipping Agency”, users of services

Director General – general responsibility, Personnel Inspector – procedure execution.

Procedure:
1. DEATH ON BOARD THE VESSEL OR IN A HOSPITAL
1.1 In case of getting information from the Client or from the Master of the vessel Personnel Inspector immediately informs the closest relatives and gets from them information as follows:
  • whether they wish lancing to be made;
  • where to sent body of the deceased (exact address, phone numbers, surname, name, patronymic of the welcomer etc.);
  • how, where and whom things of the deceased are to be sent to (exact address, phone numbers, surname, name, patronymic of the welcomer etc.);
  • how to prepare the body: to cremate; to embalm and to prepare in the proper way for delivery in a coffin.

    1.2 It is necessary to bring all information got from the relatives to the Client in writing and according to coordination with the Client information should be passed to the Agent of the vessel if necessary.

    1.3 Client or Agent in accordance with the Client’s request should forward to Limited Liability Company “Elena Shipping Agency” in the shortest possible time originals of the documents of death, detailed list of things owned by the deceased signed by the Master of the vessel and one of the officers as a witness. *

    1.4 Personnel Inspector according to coordination with the Client organizes meeting of the body in Moscow or in Saint-Petersburg through the Traveling Agent with execution of all attendant formalities and delivery of it to the meeting relatives or forwarding it father to the place of destination.

    1.5. According to coordination with the Client Personnel Inspector arranges delivery of documents necessary for receipt of correspondent compensation by successors.

    2. DECOMMISSIONING IN CASE OF ILLNESS OR TRAUMA 2.1 In case of getting information from the Client or from the Master of the vessel Personnel Inspector immediately informs the closest relatives about things happened.

    2.2 Personnel Inspector should enquire the Client of:
  • full and exact information about illness or trauma, condition of a sick person or victim;
  • full name of a medical institution, its address and contact phone numbers;
  • full list of personal things and accompanying documents left by the seaman fallen ill or suffered;
  • data of the victim’s capability including his ability to communicate with the personnel of the medical institution.

    2.3 Personnel Inspector according to coordination with the Client organizes repatriation of the victim through the Traveling Agent taking into account his condition and guided by doctors’ recommendations.

    2.4 Personnel Inspector organizes meeting of the victim in Moscow or in Saint-Petersburg with execution of all attendant formalities and delivery of the victim to the relatives or forwarding him father to the place of destination

    2.5 In accordance with the Client’s requirement Personnel Inspector organizes further treatment for the victim taking into account recommendations of attending doctors.

    2.6 According to coordination with the Client Personnel Inspector arranges delivery of documents confirming medical treatment at the place of residence or documents confirming victim’s right to getting compensation.

    IN ALL CASES MENTIONED ABOVE DIRECTOR GENERAL INFORMS ADMINISTRATION OF THE CENTRAL BASIN HOSPITAL IN WRITING ABOUT THINGS HAPPENED AND AFTER GETTING THE ANSWER ACTS IN ACCORDANCE WITH REGULATIONS CMK-001/2003 AND PROCEDURE 04 “PROCEDURE OF CONTROL OVER SUPPLIERS”
